How much does the program cost?

To start, the program costs $350, which includes:

  • A consultation with your prescribing provider
  • A consultation with a registered dietitian
  • A 2-week follow-up visit

Ongoing visits:

  • $150/month for provider follow-ups
  • $75/month for dietitian visits

Note: This pricing is for self-pay patients. If you have insurance, we’re happy to bill your visits accordingly.

Self-pay:
We work with two local compounding pharmacies that offer semaglutide and tirzepatide at reduced rates—$85–$310/month, depending on your dose. You pay the pharmacy directly.

Insurance:
Coverage varies widely—costs may range up to $1,200/month or more. Manufacturer coupons or switching to compounded medication may significantly reduce your cost.

 

No. Eviva provides a prescription only. Your medication is filled and in some cases shipped by the pharmacy.

 

After your initial consultation and 2-week follow-up, most patients are seen:

  • Monthly during dose adjustments

  • Every 3 months once your medication and progress are stable

 

To request a refill, please contact your pharmacy directly. They will send an electronic refill request to Eviva for processing.
